There are three ways to get your SCIS:

Eligibility
To apply for a SCIS, you must be registered under the Indian Act as well as be:
- 16 years of age or older; or
- 15 years of age and younger with a registered parent/guardian.

If you were previously issued an SCIS (as an adult applicant), you can use this for your primary identification if it is valid (renewal no earlier than 1 year before expiry).
When you come to your appointment/clinic, you will need to bring your identification. Your ID must contain the following criteria:
◦Your name
◦Date of birth
◦Your photo
◦Your signature
If you were previously issued an SCIS (as an adult applicant), you can use this for your primary identification if it is valid (renewal no earlier than 1 year before expiry).
When you come to your appointment/clinic, you will need to bring your identification. Your ID must contain the following criteria:
- Be issued by the provincial, territorial, federal, or state government authority; and
- Be valid (not expired).

If you currently have a Certificate of Indian Status (CIS) but were previously issued a Secured Certificate of Indian Status as an adult applicant (you were 16 or older when you applied for it), you may provide your CIS and a Guarantor Declaration Form.
Applying for a child, identification is the same criterion as above but must be the parent/legal guardian identification.

- Canadian Military Identification
- Canadian citizenship card issued prior to Feb 1, 2012
- US permanent resident card
- Identification card issued by province or territory
- Canadian permanent resident card with signature (issued before February 4, 2012)
- Provincial, territorial, or federal Correctional Services identification (with or without signature)

- Health card issued by a province or territory
- US passport card
- Government employee identification card
- NEXUS or FAST card
- US Old Age Security
- Canadian permanent resident card without signature(issued after February 4, 2012)
- Firearms license

- A Guarantor Declaration form
- Your Certificate of Indian Status
- Your Secure Certificate of Indian status that your parent or legal guardian applied for on your behalf
