To apply for a SCIS, you must be Registered Under the Indian Act as well as be:

To apply for a SCIS, you must be Registered Under the Indian Act as well as be:
- 16 years or age or older; or
- 15 years of age and younger with a parent or guardian.

What you need:
SCIS Application Requirements
Secured Certificate of Indian Status Card:
If you were previously issued an SCIS (as an adult applicant), you can use this for your primary identification if it is valid (renewal no earlier than a year before expiry)
When you come to your appointment/clinic, you will need to bring your identification. Your ID must contain the following criteria:
◦ Your name
◦ Date of birth
◦ Your photo
◦ Your signature
If you were previously issued an SCIS (as an adult applicant), you can use this for your primary identification if it is valid (renewal no earlier than a year before expiry)
When you come to your appointment/clinic, you will need to bring your identification. Your ID must contain the following criteria:
- Be issued by the provincial, territorial, federal, or state government authority; and
- Be valid (not expired).

◦ Date of birth
◦ Your photo
◦ Your signature
Registration Under the Indian Act Requirements
Registration Under the Indian Act:
When you come to your appointment/clinic, you will need to bring your proof of birth document, and a supporting government issued identification.
Your birth document must contain the following core identity information:
If applying for a child both parents must be present at the time of the appointment/event, or the non-applying parent may sign the application prior to.
Damaged Birth Documents:
The core identity information must be visible and undamaged. The birth document will not be accepted if all three of the following criteria are missing or illegible:
• Certificate Number
• Birth Registration Number
• Date of Issuance
When you come to your appointment/clinic, you will need to bring your proof of birth document, and a supporting government issued identification.

- Must contain parental information (at least one parent stated on the birth document)
- Must be issued by a government authority, (provincial/territorial/federal government, vital statistics)
- Must be issued in English or French (or accompanied by a professionally translated version)
- Must contain the individual’s personal information (name, date of birth, place of birth, date of issuance, birth registration number, and certificate number)
If applying for a child both parents must be present at the time of the appointment/event, or the non-applying parent may sign the application prior to.

If you currently have a Certificate of Indian Status (CIS) but were previously issued a Secured Certificate of Indian Status as an adult applicant (you were 16 or older when you applied for it), you may provide your CIS and a Guarantor Declaration Form.
Applying for a child, identification is the same criterion as above but must be the parent/legal guardian identification.

- Your Secure Certificate of Indian status that your parent or legal guardian applied for on your behalf
